Welcome! At Peace of Mind Therapy, our mission is to help you create meaningful connection with a supportive therapist. A professional who will customize a therapy experience just for you. From the first contact you make, we will be working with you to connect you with the right therapist and services based on your needs. If you’re ready to come see us, let’s take a look at the next steps:
Step 1
Complete New Client Form
Click the appropriate link below and help us get to know you and your goals for therapy. We will thoughtfully use this information to make you a strong therapist match.
Step 2
Get Matched With a Therapist
Based on your interests and therapy goals, we’ll identify which of our therapists may be the best fit. Then we will connect with you by phone or email.
Step 3
Schedule Your First Session
Once you have been matched with a therapist and successfully submitted your intake paperwork, our office will help you schedule your first appointment.
New Client Questionnaire
To protect your personal health information, please select the appropriate questionnaire below and follow the instructions to verify your email address. You will then receive a link to complete your form via our secure platform.
Please Note:
Our office staff are dedicated to arranging a timely match for accepted new clients to their therapist. Because clinician schedules/caseloads vary month-to month, we can only guarantee confirmed new clients an intake appointment if they call/email to schedule within 30 days of their confirmed therapist match.
If you or someone you know is currently suicidal, call 911 or go to the nearest emergency room. To speak with someone now, call the National Suicide Prevention Lifeline, a free, 24/7 hotline answered by trained crisis workers: DIAL 988.

A Person-Centered Approach
Often times people want change but aren’t sure how to do it by themselves. At Peace of Mind Therapy, we take a person-centered approach to mental health so people can gain a deeper sense of self as they face life’s twists and turns. No two experiences are alike, but we’re excited to partner with you as your process the challenges you’ve experienced.
