    Complete the applicable form from the list below to help us get to know you and your goals. This information will help us make you a strong therapist match.

  • Get Matched with a Therapist

    Based on your interests, needs and therapy goals, we will identify the therapist who may be the best fit for you. Then we will connect with you by phone or email.

  • Schedule Your First Session

    Once you have been matched with a therapist and have successfully submitted your intake paperwork, our office will help you schedule your first appointment.

We are dedicated to arranging a timely therapist match for our accepted new clients. Clinician schedules and caseloads vary month-to month, but we guarantee our confirmed new clients an appointment if they call or email to schedule within 30 days of receiving their confirmed therapist match.
