This form will help us gather basic information about you and your goals for therapy.

Get Matched to a Therapist
Based on your needs, we’ll identify which of our therapists will be the best fit for you.
Step 3
Schedule Your First Session
Once you have been matched with a therapist and completed your intake paperwork our office will assist you in scheduling your first appointment.

A note about about our office hours (due to COVID-19):
Our office staff will respond to submitted questionnaires, calls and/or emails Monday through Friday between the hours of 9:00am-2:00pm. Please expect 24-48 hours return of communication. Thank you.
